In 2014, Dr. Cook was awarded an Ontario Innovation Fund grant for “The Three Wishes Demonstration Project”. This project went on to be implemented nationally and internationally and has now been recognized by the Gairdner Foundation as an outstanding interprofessional model for end-of-life care.
